CRETINISM ( CRETINOID GOITRE ) Definition:
Congenital deficiency of thyroid function with either aplasia of thyroid or goitrous gland (Cretenoid goitre) In cases of
1. Enzyme deficiency e.g. Penderd’s syndrome, cretinoid goiter with dwarfism and deafness, due to deficiency of peroxidase enzyme.
2. Pregnant female took anti thyroid drug.
Clinical Picture:
Delayed milestones.
Supraclavicular pads of fat
Delayed epiphyseal ossification.
Very low basal metabolic rate.
In adolescence the patient will be dwarfed and mentally retarded with dry wrinkled skin. Manifestations Of Cretinism 1- Big buccal pad of fat, 2- depressed nose,
3- Thick lips, 4- protruded tongue, 5- pot belly abdomen, 6- umbilical hernia, 7- Thick short hands, 8- dry wrinkled skin, 9- coarse hair.
Treatment:
Thyroid extract daily for life.
In cretinoid goitre ® partial thyroidectomy to reduce the size of the swelling.
