SQUAMOUS CELL CARCINOMA "EPITHELIOMA" Definition: Malignant tumour arising from the prickle cell layer of the epidermis. "epidermoid carcinoma". Incidence: middle aged & elderly men. Common Sites: any skin or mucous membrane lined by stratified squamous epithelium. Commonest in the face especially the lower lip. Precancerous lesions: 1- Solar dermatitis. 2- Old scar or ulcer Marjolin 's ulcer. 3- Post irradiation dermatitis. 4- Papillomas. 5- Xeroderma pigmentosa. 6- Rodent ulcer. 7- Lupus vulgaris (T.B. skin nodules). Pathology & Cl. picture: Mic: Cell nests = keratin in the center of mass of prickle cells showing malignant criteria. Mac: The ulcer commonly in the lower lip with rapid growth showing raised everted edge, fixed indurated base and necrotic floor. Spread: mainly direct & lymphatic spread rarely blood spread. ؤ Sq. Cell carcinoma in the skin is well differentiated. than that of int. organs e.g. esophagus, with better prognosis in skin than esophagus. Treatment: 1- Excision with safety margin then plastic reconstruction. Lymph nodes block dissection is not done except if it is involved. 2- Irradiation gives satisfactory results in some cases. It has the same indications and the contraindictions of rodent ulcer. 3- Electrodesiccation may be used in lesion less than 1cm and in elderly patients. 4- Mohs' surgery can be applied and is successful in treatment.
